Level 2 - Children are concerned about mental and/or physical decline - requires 24/7 monitoring.
Level 3 - Parents are housebound with diagnosed mental disease and/or physical disibilities. - requires parent and caregiver monitoring 24/7
Background

As chronic diseases continue to be a burden on society, technology is being used more and more. About half of all adults suffer from one or more chronic diseases, which account for seven of 10 deaths and 86% of U.S. health-care costs. But preventing and treating such ailments requires time that doctors don’t have in brief office visits, and a degree of daily self-management that many patients have been unable to achieve. They often become overwhelmed by the demands of their daily regimens, slip back into poor health habits, fail to take their medications correctly—and end up in the emergency room.
Technology is offering a new fix for one of the most confounding health-care challenges: getting patients with chronic disease to take better care of themselves.

FAQs
- How many patients can your solution serve?
Unlimited. There is no limit to the number of patients Aptus can manage. It is easy to scale up and down. The provider will have access to one portal that they can brand and customize, as needed, to quickly see stats for all the patients in one place.
- What’s the cost of your solution and the ROI?
Our cost averages to be $120 a month per patient. The revenue ROI back to the provider is roughly $300 per patient. The larger the patient population, the higher the reimbursement amount. The amount received back is immediate for the current patient population of any size. Providers get reimbursed directly from CMS through our billing and reconciliation engine. For the patient, they are only billed their regular copay.
- What are the onboarding requirements for new practices?
Onboarding is simple. Our team will need to know your desired timing and establish a point of contact for setup and ongoing reporting. We ask what financial tools you use for faster integrations. We also need to know EMR and data integrations. Together, we go over compliance and patient privacy.
- Can your solution be integrated with our organization’s EHR?
Yes. We are device and system agnostic. Our team truly tries to make this as simple as possible for the provider. Aptus is here to enhance your daily operations, not to replace anything that you are currently doing.
- What insights can physicians get from Aptus?
Physicians get real-time access to all of the patient biometrics, which is automatically sent for provider and family. Typically, physicians will not receive data from the device unless the patient is in the office. With Aptus, they can receive timely alerts when something is out of the medical norm.
- How does your solution help the provider to improve patient outcomes?
Since we are working together with the provider we are able to monitor all of their vitals in real-time alleviating a lot of the guesswork and keeping them in the home longer and out of the hospital. A provider can easily access any patient's account and specific documents and history associated with that account via keywords. It is a one-portal view of your whole patient population.
- Does your solution include medical devices?
We have a list of over 150 devices that can be used that are both Bluetooth and cellular. Depending on the volume, we can look at customized devices and add them to our platform. We can provide the devices or you use what was purchased by the provider.
- Who ships the devices to patients’ homes?
Our team does all of the medical device fulfillment. We can ship devices directly to the provider or the patient.
- How do you secure patient data?
All data is secure. We are 100% HIPAA compliant, and all data is encrypted to protect the patient.
- Does Aptus replace my home health care team?
Aptus does not replace your home health care team but improves efficiency and profitability by notifying you when and where somebody needs you eliminating unnecessary work and travel.
