Physician Education Hub

Sharing important updates from hepatology, radiology, oncology, and the role of noninvasive diagnostics in these fields.

Welcome back to our Physician Education Hub, and greetings from Chicago. I’ve been at RSNA many times, and in recent years I have witnessed the growth in imaging technologies directed at the important challenge of diagnosing and treating liver disease. This year has been no exception, with notable developments in the applications of MRI, PET/CT, and ultrasound for the detection and diagnosis of both chronic liver disease and cancer (e.g., LI-RADS), highlighting the vital role radiology plays in this field. There have also been some promising developments in the utility of MRI in treatment planning and monitoring the effectiveness of interventions (e.g., Yttrium-90 therapy) in patients with liver cancer.

Highlighted Article

Delving deeper into these exciting times for liver imaging, an area of particular interest at RSNA, is the growing role of artificial intelligence (AI) in gastrointestinal radiology, which is covered in several sessions. On this topic, I’d like to introduce a paper recently published in Applied Radiology, where Dr. Matt Kelly walks us through how AI and convolutional neural network (CNN)-based approaches can save radiology time while also reducing subjectivity and inter-reader variability associated with conventional techniques. For example, AI- and CNN-based approaches have been used successfully in liver volumetry and quantitative three-dimensional reconstruction of the biliary tree from MRI (see Figure 1). The article also recognizes the need for widespread clinical adoption of AI in regulatory-cleared products to enable seamless integration into the radiology workflow. This should bridge the gap to the clinic and address challenges in liver disease management, imaging chronic liver diseases, and liver cancer, while ensuring that the role of radiology in managing these conditions continues to evolve. -Dr. Carlos Duncker READ THE ARTICLE READ PREVIOUS ARTICLES

MRCP+

FDA gives thumbs-up for Perspectum Diagnostics' biliary visualization software

This ground-breaking new quantitative imaging tool uses artificial intelligence led technology to enhance MRCP images. MRCP+ creates true 3D rendered models of enhanced data and metrics to facilitate visualization and quantitative assessment of the biliary tree and pancreatic duct.

An advanced biliary visualization software that:

•Quantitatively analyses the biliary tree and pancreatic duct
•Uses a noninvasive method without contrast
•Can be generated using historical MRCP images
•Requires no additional hardware, reducing capital expenditure

What is MRCP+?

MRCP+ is a noninvasive diagnostic tool that calculates quantitative 3D biliary system models.1 It enables the measurement of bile duct widths and automatic detection of regions of variation. MRCP+ includes tools for interactive segmentation and labelling of the biliary system and structures. It allows for regional volumetric analysis of the biliary tree, pancreatic duct, and gallbladder.
Combining image viewing, processing, and reporting tools, the quantitative metrics provided are designed to support physicians in the visualization, evaluation, and reporting of hepatobiliary structures.
This information is provided through software-as-a-service in an easy-to-read report delivered directly to the radiologists or referring clinicians.
MRCP+ is available clinically, with FDA 510(k) clearance and CE marking.

The American Medical Association (AMA) issued two Current Procedural Terminology (CPT) codes for the Perspectum MRCP+.

The following Category III CPT codes may be reported by physicians, hospitals for outpatient services, Independent Diagnostic Testing Facilities and Ambulatory Surgery Centers: 0723T and +0724T.

CoverScan

The world’s first quantitative imaging solution for the assessment of multi-organ tissue characteristics, function, and size using a single, non-contrast MRI scan. This noninvasive technology can support clinical decision-making by quantifying metrics from images of the heart, liver, lungs, kidneys, pancreas, and spleen.

What is CoverScan?

CoverScan is a tool for quantifying the characteristics of six different organs using a single noninvasive scan, enabling clinicians to visualize and assess multi-organ health. Referring clinicians are provided with a report of 49 metrics from multiparametric MR data for easy assessment of multi-organ function relative to reference ranges.
The information provided by CoverScan, in conjunction with other tests and clinical evaluation of the patient, may form part of a clinical decision-making pathway and enable physicians to make more informed clinical decisions for patients. Importantly, repeated analyses using CoverScan may aid clinicians to monitor patients and personalize treatments.
CoverScan is an advanced cloud-based software suite, delivered as a software as a service solution, and uses existing clinical MR infrastructure.
In the U.K., the multiparametric multi-organ imaging technology is available as a tool to aid diagnoses for patients with multi-systemic diseases such as long COVID (1) or insulin resistance (e.g., metabolic syndrome and diabetes mellitus). It has been available within the U.K.’s NHS since January 2021, under the MHRA's temporary authorization, in response to the COVID-19 pandemic. CoverScan 1.0 (Siemens 1.5T and GE 1.5T) received full UKCA clearance in May 2022.

Couinaud segment analysis

Liver fibro-inflammation (cT1), fat (PDFF) and volume are quantified for individual Couinaud segments. This can assist clinicians in accurately predicting the future liver remnant (FLR) to inform surgical decision making.

Figure 1:  Quantitative multiparametric MRI allows safe surgical planning in patients undergoing liver resection for colorectal liver metastases: Report of two patients.3

Patient 1

  • Hepatica indicated high fibro-inflammation (cT1) and fat (PDFF)
  • Future liver remnant volume: 23%
  • Post-hepatectomy liver failure (PHLF)
  • Information may have changed surgical plan, averting PHLF that delayed the patient’s recovery

Patient 2

  • Hepatica indicated normal fibro-inflammation (cT1) and fat (PDFF)
  • Future liver remnant volume: 29%
  • Uneventful post-operative course and short hospital stay
  • Information would justify surgical plan
